Hi everyone, I am a research nurse in Italy, in the field of type 1 and type 2 diabetes and cardiovascular diseases. I would like to know if some of you use data collection sheet, or patient record/database, so I want to say a documentation (source documentation) where you write all the activity that you make with patient during the Site Visit, performed in accord with the protocol and the Sponsor..
